Michigan Institute for Educational Management (MIEM) has partnered with the Michigan Department of Education (MDE) and The Center for Educational Performance and Information (CEPI) to bring a two-part series on "Accessing and Properly Filling out the New Electronic SE-4094 form."
This form is required to be submitted no later than October 8, 2007. It's very important to complete this form on time because your entire State Aid payment will be withheld until it's completed.
Each part of this series is about 15 minutes long and should provide you with all the information you need to successfully fill out your SE-4094. If you should have questions after viewing these, contact the responsible person provided in the video.
